I.
Sayaka wishes that Kyousuke would understand what she feels for him.
The next time she visits him in the hospital, he asks her point-blank if she is in love with him. She says that she is. He nods his head and explains that he really only thinks of her as a friend, that he doesn't want to take advantage of her, that he's so sorry.
II.
Sayaka wishes that Hitomi would break up with Kyousuke.
When Kyousuke tells her what happened from his hospital bed, heartbroken, she makes her first move. She confesses her love a week later, and he agrees to a relationship with her. They share their first kiss with Sayaka leaning over him. Two weeks after that, he tells her that he's not sure how he feels, that everything in his head has been a mess since Hitomi dumped him, that he's not sure that he's really in love with her.
III.
Sayaka wishes that Kyousuke could play music again.
His hand never heals right enough for him to play his violin, but he can hold on to a mallet just fine. He learns to play the xylophone, picking it up with shocking speed. He becomes so consumed in his music that he doesn't have time for anyone anymore...
IV.
Sayaka wishes that she could play like Kyousuke used to be able to.
Everyone is shocked and impressed by her skill, how readily she picks up music. She attributes it to listening to so much classical music with Kyousuke. At first he is proud of her, but his eyes grow sadder and sadder every time he watches her play. He stockpiles medication for a few days and then swallows all of it at once.
V.
Sayaka wishes that Kyousuke would have to rely on her.
The injuries in his legs become infected, and he develops a fever that almost takes his life. He is made well again, but now none of his body works the way it once did. She feeds him rice in the hospital. When he thanks her, looking embarrassed, the guilt surges through her.
VI.
Sayaka wishes that Kyousuke would fall in love with her.
Their first date is in the hospital cafeteria. They eat pudding together and laugh. It feels so beautiful it doesn't feel real. It never feels real, because she knows that he would never have loved her if she hadn't made her wish.
VII.
Sayaka wishes that she was not in love anymore, and that she would never fall in love again.
She isn't, and she doesn't. She dies in a battle against a Witch. She was inattentive. After her wish, she hadn't cared about much.
