You could make an epilogue, where she makes the formula and Ichigo can change back at forth(between boy and girl) at will. That way they can have all kinds of kinky fun and one day get pregnant. Heck Orihime can take it too; then she can change to a guy for a while, so they can have reverse actionOri-boy, Ichi-girl). Then they could do whatever yuri, yaoi Ori-guy&Ichi-girl, Ori-girl & Ichi-boy. Heck Ichi could also get pregrers from Orihime and not just Orihime from him. All kinds of kinkiness. Please consider this.

Why such a complicated end? Orihime's powers could fix them (Ichigo's gender, her lactation) in a matter of minutes.
