Five Major Plagues of the World: Slaughter—the embodiment of war. As a cat, it appears as a huge, bloodstained beast with bloodlust in its eyes and gigantic claws. As a human, it appears as a tall, tan-skinned military man with bloody teeth and no other facial features.
Hunger—the embodiment of starvation and hunger. As a cat, it appears bone-thin and eyeless, with a ragged, matted dull gray-brown pelt. As a human, it appears as a darker-skinned bone-thin child with a distended stomach and, once again, lacks eyes.
Malady—the embodiment of disease. As a cat, it appears large and slack-jawed with its matted fur rotting off and green, unwholesome eyes milky and blind. As a human, it appears as a large and white-skinned adult with no hair, milky eyes, and various large tumors across it's body.
Despair—the embodiment of hopelessness. As a cat, it appears shrouded in darkness and radiates an aura of despair, with two completely white eyes staring out of it's black cloud. As a human, it appears nearly the same way, but, you know, humanoid.
Death—the embodiment of death. Reaper of souls, etc. Appears different to everyone, depending on your views of death. Those who are afraid of death could see the Grim Reaper, those who long for death as an angel, etc.

How Each Cat Saw Death

Snow—Sam/Wolf, for reasons stated before.

Wolf—a huge cat with giant claws and fangs. Wolf always wanted to die in battle over anything else, but she was still afraid of death (like most are). Here she saw the only opponent who could beat her.

Lion's Roar—himself, sick and old. As Lionblaze, he had always feared sickness and old age, because he could not be beat in battle. He knew that when he did die, it would not be quickly, in battle, but slowly, from illness or old age.

Jay's Wing—a drowned cat. Ever since he failed to rescue Flametail, he was terrified of drowning, and his worst death nightmares were of him drowning.

Dove's Wing—a dark black-furred cat, the standard image of Death (how it appears to most). She feared death because of her previous immortality.

This was a bit of a flop, but here it is anyway.

Question: Do you like cake? (Skye)
Gingeh: No way, cake is gross. I hate frosting, and most of the bread stuff tastes way too sweet.
Annie: I prefer pies or cheesecakes.
Ryan: Cake is the best thing I ever had as a human. It's not my fault that Annie doesn't like it much, but just how sweet it is...mmm...better than mice anytime.
