The information within this file pertains to a single patient and the handler that has been assigned to said patient.

-Pain = Love-

Chiko "Tenshi" Hinanawi, is the daughter of a couple that became members of high society after winning the lottery. She prefers to be referred to as Tenshi, so out of consideration that will be the name used for this document. She was placed under our care after her parents stated that they could no longer cope with her misbehaving in school. Said misbehavior was inflicting strange punishments upon herself, this has caused her to be referred to as a delinquent by her school's faculty. From what little information that was provided we have found that, in the past Tenshi would inflict punishments of various degrees on herself. Her self inflicted punishments have left her with a high pain tolerance, Tenshi herself doesn't seem to understand how her behavior is strange and/or disturbing to those around her.

Examples of her punishments: some were very simple like giving herself spankings where as others were more… extreme… such as one case where Tenshi had just been recently admitted to the hospital. She had not left her room for hours and had locked the door leaving the staff worried that something might of happened to her. Using the spares keys we were able to enter the room where we found Tenshi, her whole body was bound by rope in a position that many would acquitted to something seen in a bondage session. We quickly untied her fearing for her health and the possibility that someone may have assaulted her. When asked who had bound her Tenshi admitted that she had done it to herself. We were understandable skeptical at first but Tenshi proved she was telling the truth when she picked up the rope we had just unraveled and in a fashion similar to a dance she was once again restrained in the same position we had found her. After once again freeing her, the nurses took her to one of the observation room with the decision that her consoling sessions would start that same day, Dr. Namazu was choosing for Tenshi's session.

Dr. Namazu asks stroking his long whisker like mustache: "Tenshi do you like being here?"

Tenshi responds joyfully: "Yes!"

Dr. Namazu continues stroking his whiskers: "Why do you like being here Tenshi?"

Tenshi answers smiling: "Because it's so much fun, there are so many bright and interesting people here. Someone as lowly and pathetic as myself is lucky to even be here."

Dr. Namazu continues the conversation with Tenshi answering the questions he is asked.

Dr. Namazu informed us that he had deliberately avoided asking questions with regards to Tenshi "habits" saying that it was important to understand the patient's personality and personal views before confronting them about their aliments, a statement the rest of the staff agree with. While we were happy that her condition hadn't seemed to affected her social skills, none of her answers gave us any kind of insight to what her condition is or its cause.

Reviewing the transcript in its entirety we grow more worried for Tenshi, due to the manner in which Tenshi refers to herself. Tenshi refers to others using normal words, phases, and even compliments, but when she is speaking about herself she uses insults, degrading remakes and hurtful expressions. Some might say that Tenshi is subjecting herself to verbal punishment.

When asked when or why she started feeling this inclination to punish herself, Tenshi replied "For as long as my tiny, feeble mind can remember I've was always been like this". Since asking about the origin of her condition yielded no results we questioned her on what happened in her school. Her replied that she felt the school's punishments were lacking and so to save them the trouble she would punish herself, the odd thing is that Tenshi had no record of every misbehaving. A high possibility is that since Tenshi never misbehaved she was never punish, and for someone like her punishments had become a constant in her daily life and the act of going long periods of time without punishment must of left her feeling a kind of "withdrawal" that distracted her from her studies. Which we came to the conclusion that she punished herself out of guilt for being distracted and also she could get her "fix" of punishment.

After a month we still were unable to figure out what could have possible been the cause for Tenshi's abnormal masochistic behavior... that is until we taped Tenshi's parents most recent visit to the hospital. For information see -Video Playback- in the next file.

With what was shown we have come to the conclusion that Tenshi asking for punishment is derived from a mental complex that being hurt by others equals being loved by others which developed from Tenshi's deeper rooted emotional problems which stem from the lonely life she had with her parents which has left her emotionally starved for attention and acknowledgement of any kind.

Ironically despite all the information in this document, Tenshi is surprisingly in many ways a very innocent girl. She holds no ill will towards anyone in the hospital be them patient or staff member, she participates in the games, can hold conversations with others and actually wishes to befriend the other patients. Although because of her innocent mental state she sometimes ends up insulting the other patients causing them to retaliate but giving her masochistic tendencies Tenshi feels more like they are playing with her instead of hurting/insulting her.

Since Tenshi is allowed to leave the hospital (and technically leave the imaginary world the girls have created) the other patients say that Tenshi goes to "heaven", of course since she returns Tenshi is considered a celestial being by the rest of the patients freely moving between "heaven" and the imaginary world they live in. Many of the others patients say that Tenshi has the power to cause quakes, which is not just restricted to earthquakes but also quakes in the metaphorically sense. Such as causing others "quaking with angry" because of her innocent yet insulting words or making them to "quake with fear" when Tenshi starts asking for them to punish her, etc.

Curing Tenshi will not be easy, conditions that are centered around attention seeking are tricky to deal with to say the least. We will have to find way to wean Tenshi away from her masochistic antics while also helping her to distinguish and realize the difference between positive and negative attention, this will prove difficult. Also some of the doctors think that Tenshi isn't just attention staved but they also believe that Tenshi thinks that any kind of attention she receives validates her very existence, if this is true then we must take care in our methods since given Tenshi's mental state one false move could cause Tenshi to resort to even more drastic or even life threatening methods of attention seeking. We will not let that happen.


-Don't be so Koi-

Since the arrival of Reimu and Marisa our hospital seems to have become the go-to place for patients with strange and/or peculiar medical conditions. Because of this we have also received numerous resumes of medical students applying to be a part of the hospital staff. Most of them apply with hopes of making a name for themselves in the medical world by curing our patients, others out of fascination of the patients unusual conditions, and there are also those that wish to test various methods of treatment to see if they can help in any way.

One such person is Iku Nagae (she prefers to simply be called Miss Iku). Who employs both Dance Therapy and Music Therapy as methods for treating the patients.

Dance/movement therapy, (DMT) or dance therapy is the psychotherapeutic use of movement and dance to support intellectual, emotional, and motor functions of the body. As a form of expressive therapy, DMT looks at the correlation between movement and emotion. A typical DMT session has four main stages: Preparation (the warm-up stage, safety is established), Incubation (relaxed, let go of conscious control, movements become symbolic), Illumination (meanings become apparent, can have positive and negative effects), and Evaluation (discuss significance of the process, prepare to end therapy).

Music therapy is the use of interventions to accomplish individual goals within a therapeutic relationship by a professional who has completed an approved music therapy program. Music therapy is an allied health profession and one of the expressive therapies, consisting of a process in which a music therapist uses music and all of its facets—physical, emotional, mental, social, aesthetic, and spiritual—to help clients improve their physical and mental health. Music therapists primarily help clients improve their health in several domains, such as cognitive functioning, motor skills, emotional development, social skills, and quality of life, by using music experiences such as free improvisation, singing, and listening to, discussing, and moving to music to achieve treatment goals.

Giving Tenshi's social standing (and at the insistence of her parents) she is allowed to return to her home once a month. Miss Iku and by extension the rest of the staff were indifferent to this arrangement, until Miss Iku filed a report stated that after her most recent trip home Tenshi had returned and there were whipping mark on her back and rope burns on her upper arms. Many of us believed that Tenshi had "punished" herself while at home but we quickly changed our minds when Miss Iku informed us that she had asked on sight forensics group to look at Tenshi marks and the results were puzzling. Which was that both types of injuries were fresh and were inflicted at roughly the same time, Miss Iku also pointed out that if Tenshi arms were tied then how could she have possible whipped herself. Miss Iku fearing that Tenshi may suffer more injuries requested that she was to accompany Tenshi on her next time home, which was granted the same.

The first and only time Miss Iku went with Tenshi for one of her visits home, she returned dishearten. When asked of what she thought of Tenshi's parents she simply replied "I have seen ice with more warmth then those people" submitted a brief summary with what little information Tenshi's parents provided and promptly returned to her duties. Miss Iku wrote that Tenshi's parents spent most of their visit ignoring Tenshi, dodging questions, and any answer they gave was vague and unhelpful. This didn't provide us we much help, so we decided that the next visit was to be held here in the hospital and recorded on video.

For most of the conversation Tenshi's parents avoided answering any questions while insulting the hospital's staff and the other patients which wore on Miss Iku's nerves as it did for all viewing the playback. Things came to a boil when Tenshi replied to one of Miss Iku's questions and her Father reacted most rashly.

Mr. Hinanawi slapped Tenshi sending her to the ground shouting "WHO SAID YOU COULD TALK?!". Tenshi quickly picked herself up and looked at her father with what some would call the happiest smile they had ever seen on Tenshi's face.

Miss Iku rushed to check Tenshi seeing if she was busied or if worse bleeding, the words that came out of Tenshi's mouth gave all who were viewing the video a much clearer idea on what the main cause of Tenshi condition could have been "Did you see Miss Iku? Papa noticed me!"

Surprisingly while Miss Iku was tending to Tenshi, Tenshi's father spoke in a very haughty and demeaning tone. "We thought you doctors were going to find out what was wrong with her so you could fix her, but all I see is you just coddling her."

Speaking up Mrs. Hinanawi added "Quite true, maybe your whole hospital is nothing but a sham." Smiling smugly as she fanned herself.

The tape showed that at this point Miss Iku was fed up with Tenshi's parents, she replied in a very flat tone "I will remind you, that you are the ones who admit Tenshi into our care and since then you have pretty much abandoned her here." If one looked carefully enough at the room's thermometer they would have noticed that it had dropped about several degrees.

Tenshi's father reached into his pocket pulling out a cigar and a lighter, placing the cigar in his mouth he said "Our word against yours, and we have money to back up our words." Flicking his lighter open with the intention of lighting his cigar.

Looking at Tenshi's parents who were looking quite smug Miss Iku walked over to the father, yanking the cigar out of his mouth and lighter from his hand stating in a harsh cold tone "Smoking is prohibited in hospital." Before the man could reply Miss Iku simply asked "You do know that all visits are recorded?"

Upon hearing that both Mr. Hinanawi and Mrs. Hinanawi stood up yelling "WE NEVER AGREED TO THAT!"

Speaking in an even colder voice (and the room felt just as cold) Miss Iku spoke "First, you did. Second its standard procedure, all hospitals are required to record and copy all visits and documents pertaining to a patient. For in the event a patient is transferred from one hospital to another, the patient's prior hospital is required to provide said information to insure the patients continued care and health." Tenshi's parents appeared rooted to the floor as Tenshi innocently looked on like she was watch some kind of show, Miss Iku drop the cigar she had confiscated into the trash bin. Facing them, she said "Now… if either one of you ever threaten Gensokyo again, we will notify the police and you WILL be charged with child neglect and abuse!"

"THATS BLACKMAIL!" Mrs. Hinanawi shrieked.

Miss Iku walked over to Tenshi's father who at this point was silent and sweating profusely "Nope" Miss Iku said grabbing his hand, holding it open she placed the lighter in his hand "Just a friendly reminder…"

After being confronted by Miss Iku and becoming slightly phobic of her, Tenshi's parents admitted that they were fully aware that they were the causes for her behavior, and that they had every intention of trying to shift the blame onto someone or something (in this case our hospital) for their daughter's condition.

Now that we know the cause and have a better understanding of Tenshi's condition we can start on trying to help her.

Miss Iku was assigned to oversee Tenshi since we hypothesized that by undergoing Music and Dance therapy Tenshi would learn what healthy physical stimulation is and we secretly hoped that Tenshi would be left too tried from her therapy to try and inflict any punishment on herself. Sometimes Miss Iku acts more like Tenshi's adoptive older sister then her handler, which we hope that by having positive (non-abusive) interaction with someone who she sees as family will help Tenshi to start realizing that there is a difference between positive and negative attention and hopefully she will realize that being hurt by someone does not equal being loved by them. Even if Tenshi's antics more often then not leaves her frustrated, Miss Iku is determined to help Tenshi.

Beside being Tenshi' handler Miss Iku also works in the hospital's mail room soughting out the days mail and swiftly delivering it. She acts as a sought of "personal messenger" for Tenshi to the other patients, mostly that Tenshi wishes to speak and/or play with said patient.

When playing pretend with the patients Miss Iku is an Oarfish youkai, since many of the patients have come to associated Miss Iku with an oarfish, mostly likely that Miss Iku feeds the oarfish in the hospital's rather large pond, and the fact that Miss Iku is normally seen wearing a white scarf and shawl with a red frill which heavily resemble oarfish tails and fins.

On a side note, Miss Iku can some times be caught practicing different types of dances one being flamenco which she uses for her dance therapy sessions, the other being disco although she denies it out of embarrassment.


-Doctor's Personal Notes-

The time I have spent here at Gensokyo has been quite enlightening, but now after typing Tenshi's file and seeing how she and the other patients have progressed and bonded with one another. I am starting to having some thoughts regarding our patients and I find myself asking... should we really "cure" them?

To cure someone means to heal a wound or remove an infection, but for most of the girls their "aliments" have become a core part of their personalities, a part of their very identity. Granted some of the cases would make life difficult (Suika, Yamame, the Komeiji sisters, Cirno, quickly jump to mind) but giving time could we not find ways to help them to adapt to the world even with their various conditions instead of simply "curing" them?

I wonder, would curing them mean removing a part of who they are as people just so they can fit in with what society deems as normal and acceptable? Would it be a relief for them? Could they live fulfilling lives? Would it mean having to spend their lives with people whispering about them behind their backs and being treated as if they are lesser beings? Could they find love and start families given their history here? Would it mean they would lose contact with the friends they have made here, be it by choice or outside influence? Would they thank us for "curing" them?

In essence what I want to know is that, would us curing them... make them happy?
