Everything went according to Revan's plans for the final battle, until the activation of the Mass Shadow Generator on Malachor V. Rather than destroying the bulk of the Mandalorian Armada along with the Republic troops not loyal to him, the weapon instead flings two fleets across space and time to become the newest power in Citadel space.

Battle of Malachor V
Mild panic flitted through the veins of Jedi General Meetra Surik's body. Through tactics and guile, Revan had brought nearly the entirety of the Mandalorian fleet to its knees, trapping them against the planet of Malachor V and the massive, if ragged, fleet under her command. Now though, time was beginning to show that even such a perfectly prepared trap could be defeated through raw determination and firepower. The Mandalorian fleet ahead of her was steadily clawing its way out of Malachor's gravity well, and if they broke free of its hold they would tear through her tired fleet and be able to freely attack Revan's reinforcements from behind.
And that was something she could not permit.
Taking a breath to steady herself, Meetra clasped her hands behind her back before turning her head to look at the Zabrak soldier on her bridge. She held Lieutenant Bao Dur's eyes for several second before giving him a silent nod, giving the order for him to end the battle before their eyes.
He turned away with a matching nod before deliberately typing out a series of commands on his console. When he hit enter, there was a brief pause and then the surface of Malachor V began to change. Fields of purple plant life began to shift and crack, wide oceans of blue water heaved and tossed about with massive white caps as green lightning arced away from the surface of the planet, great rippling sheets of the venomous looking energy rolling across the planet's upper atmosphere. That was the point where every alarm on every console blared to life as stars stretched before Meetra's eyes with the warping planet at the center before the entire battlecruiser heaved beneath her feet and she lost consciousness.
Meetra shook off a shiver as she opened her eyes, finding nothing immediately but the cool durasteel deck plating of the Redoubtable's bridge beneath her. Pulling her cheek off the plating, she pulled herself to her hands and knees before looking around her bridge.
"Report!" She ordered loudly, hedging that some of the other crew members were waking up as well. Fortunately she was met with a few grunts and groans as various people stirred to life. Reaching out with her Force senses, she heaved a sigh of relief as she confirmed that all of her crew were at least alive.
Looking across the bridge, Meetra heaved herself to her feet and stood, looking about before staring out the front view port roses what Malachor had done. Only what she saw wasn't the devastated remains of the planet, or the shattered hulls of the republic and Mandalorian fleets.
No, what laid before her were the intact, if unpowered, forms of both fleets, lazily drifting through space as if they had been hit by enormous ion blasts, and behind that, dominating the view was a serene blue and green planet, covered in swirls and masses of wispy white clouds that drifted slowly through the atmosphere.
