May I point out a few grammar errors in your translation (But keep in mind I am amazed at your ability. I'm a one language gal.

So here are a few corrections:

1. Using That instead of The or those. This happens in a couple of places and is an easy mistake: "That" eyes (wrong) The eyes or those eyes (correct)

2. Using an with a plural: "an" eyes (wrong an indicates singular - for example "an eye" would be correct): the eyes (correct)

3. Her mother does not want to: "carry" her to another hospital (that could be very tiring! carry means to move while supporting "the girl carried her umbrella in case it might rain later." ) she wants to: "take" her to another hospital

These are all small mistakes, nothing that takes away from the plot at all.

Now a question: Did you not have Fate introduce herself because the scene was from Fate's POV? I created a fun little scenario in my head for how it would go from Fate's POV:

"M-My name is Testarossa F-Fate" Fate says shyly.

Fate notices a look of surprise cross Nanoha's face as the girl brings her hand up to cover her mouth. After a moment a new look enter's Nanoha's blue eyes, a look Fate is unfamiliar with.

"Nyahahahaha" Nanoha gigles.

"Mou, Nanoha" Fate pouts, "Is there something funny to you about my name?"

Nanoha slowly stops giggling then in a consoling voice, "I'm sorry Fate, its just that it seems I already knew your name."

Now Fate's face had the look of surprise on it.
