Beautiful moments, I'm glad you didn't kill Cas.

Not expecting two major deaths but nice and sweet ending to underplay that

A beautiful piece, and very well crafted. The change in mood from the first part to the second part was very effective, and the contrast between the serenity of the first section and the brutality of the second, before returning to a more bittersweet but peaceful ending gave the whole thing a lot of impact. You have a good handle on the characters and I thought Cas' dialogue in particular was done well.

I liked how you handled the "I love you". On my first read through I felt it seemed too sudden, but that may be because I wasn't expecting it. Given how the opening felt like it wouldn't be out of place in canon, and I've almost accepted that Dean will never say "I love you" to Cas in canon, it came as a surprise, but a pleasant one. I think Dean's reaction to it was handled well. Oftentimes I've read fics where either he'll freak out, or take that as his cue to immediately grab Cas and kiss him, but I like how in this case his reaction was more internalised, which I think was very in character. Cas disappearing when Dean looks away was also a nice touch.

The ending in particular made me emotional. It's always been a thing for me that any fic in which Cas and Dean die and Sam gets left behind, or Sam and Dean die and Cas is left behind, or any variation on that hits me pretty hard, but the inclusion of memory!Sam in this made it even more bittersweet. The repetition of "all the time in the world" was very powerful.

Overall, I enjoyed this story very much and I think everything was pitched just right. It really captured the true essence of the relationship between Dean and Cas and your writing is a pleasure to read.

I'm not as invested in Hilson as I am Destiel, but if your House stories are written as well as this, then I definitely think they'll be worth a read.
