I just wonder why wouldn't House be a doctor anymore? That's an academic title, earned by study and research & publishing. He might not have a license to practice anymore, because he wasn't in the business for too long, but he is a doctor nonetheless. I know shows get this point wrong all the time but still... I thought I'd mention it,

This is a lovely ending but it's so sad that this great story is over.
I did some research myself regarding what House's punishment would be if he ever got caught or wanted to officially go back to the living again, which pretty much was consistent with what you said in this chapter. Only, House switching the dental records is falsification of documents for which he could get up to six months jail time or probation.
