This was well-written but I did have some trouble empathizing with the characters. I honestly wouldn't have minded if they didn't end up together.
The one thing that bothered me was how Natsuki based her positive view of Shizuru off the fact that Shizuru didn't have sex with her. Just because Shizuru backs off on her word (for example, when she says Natsuki needs to give her sex as payment for healing Mai and then says Natsuki actually doesn't after the fact) – something she shouldn't be forcing Natsuki to make a decision on in the first place– doesn't mean she's a wonderful, caring person or anything. Just because someone doesn't do what they shouldn't even do in the first place doesn't imply any sort of positive character. I am not a caring person just because I don't rape someone. It's something that should be expected of any basic, human being. Shizuru is not a caring person for not raping Natsuki, even if Natsuki was in a position where she could have easily been taken by Shizuru.
All in all I do enjoy Shiznat and this put a neat twist on it! Just wanted to say how I felt.
