One thing that could happen is that Helga would tease him/insult him/attack him one time too many. We know what Arnold is like when he loses his temper, so if Helga pushes him too far, she's going to end up in the receiving end of a roundhouse kick.

Sure, Arnold will feel terrible about it later (despite his friends' applauding him for finally standing up to the bully), and maybe he'd apologize, but it would mean Helga finally realizing that she lost Arnold forever.
