This was astoundingly well-done. Although John *did* need to hallucinate about the hound for the sake of the episode, I actually like your version of the story better. It's a nice reminder that John was a soldier, and the whole "I've killed people" quote is more than a throwaway line for the sake of black comedy. What's more is that the hallucination/flashback combination is (based on my admittedly limited understanding) realistic. John mistaking the monkey for the boy in Afghanistan was particularly chilling, and his panic after "shooting" Sherlock felt like a bullet in my heart, too.

I do have a bit of criticism though, regarding Sherlock's quick reasoning that it wasn't his fault. Yes, he felt enormously guilty about it. That (amazing) line- "If he ever sees that look on John's face again, he will put a bullet in the head of whomever's responsible. Even if it's himself."- proves it. But the fact that he thinks it wasn't his fault just feels...wrong. Sherlock may be rude, arrogant, and even borderline cruel, but at the end of the day he loves John, and surely the guilt he's dealing with would overpower the technicality of the sugar. I'm not sure if my issue lies within the plotting or within Sherlock's established character, though, so this is just me being nitpicky.

Apart from that, though, this was absolutely brilliant. It was sad and horrifying, and an unusually strong portrayal of what post-combat PTSD can do to a person. Very, very well done!
