This is a beautiful alternative ending to the movie sequence. It is apparent that the soul transfer procedure is not always guaranteed to work? Mo'at and Neytiri did hold grave fears as to whether this practice would actually work? It would be interpreted that the procedure has been carried out before by a sanctioned "Tsahik" quite successfully in the past, and it would've been taken for granted that "Jake", being of healthy mind and strong heart, should have no problem traversing over to his permanent Na'vi body? Your short story shows what could've happened, and the distress and upset his departure would've bought to bare on Neytiri and her newborn child very well! Congratulations on a very moving story.
