first of all: This fanfiction strats great! It answer questions, but it also raises new questions.
It‘s no problem for the Bear, that Gilbert is kinda OoC, the Bear means... he is in a mental asylum. He has some mental disorders and that makes him different from the „orginal“ Gilbert.

The Bear want to correct you in some German sentence: „Ich verspreche Ihnen“
- „Ihnen“ is used if you speak to a stranger or write to a person like your professor. „Du“ fits better here, since this is use for person you know (very) well
- This sentence needs an „es“ than it makes more sence.
(You also can only write „Ich verspreche“
The Bear sounds very nerdy... But he knows that, since he is a German!

Also, to come back to the good things, you discripe things very well. The Bear can imagine the asylum (or the room, where Gilbert and Lovino sleep)

Alright, I'm finished (hopefully not permanently?). I cried so much throughout this story that my eyes ached! I feel so bad for everyone but, as I said before, Gil is my favorite, so I hurt for him most. Followed by Mattie. PruCan is so awesome! I'd really love to see more from this story; I very much hope you continue it sometime.

God, I am already in tears - I can only imagine how much worse it could get. Prussia is my absolute favorite.

Okay, I'm a German mothertounge. So, do you mind if I give you a bit of advice? If you translate "I promise" you would say "Ich verspreche es (dir). "Ihnen" is a polite form of "du" which means you. You say it to teachers, adults and so on but definitely not to your brother. Hope it helped and I didn't annoy you.

Hey, so I know it's been a while since the last update, but I just want to say that I absolutely love this story! You've done the characters and story well, and I think you've portrayed the different mental illnesses well. I know you've said you might not continue this, but I would love it if you would! This story has been a fantastic read, and I'd love more of it!

I really really really love this story. I sincerely hope that someday you will find the will and motivation to pick it up again. This truly is an amazing story, one of my all time favorites and I'm sad to see it all come to an end, especially when the story is yet to be finished. I am, however, grateful for all the laughs and tears you've evoked in me during this amazing journey they call "Welcome To My Life". Against all odds I shall continue to harbor hope for the continuation of this fic. Thank you. I can't wait to see more of your work.

So I may have read this chapter in the middle of the night because I couldn't stop reading and I may have cried my eyes out. Maybe. I also may have three hours to sleep until I have to wake up and go to classes. Just maybe. It could also be that I have plans in the evening and sleep deprivation is really not a good idea. Wow, I really thought this fic was abandoned. I feel both happy that you updated, but sad that you might not continue. I think this is such a good story that really deserves a proper ending if only just to tie up some loose ends. I know I could use the closure, haha.
That being said, I think this chapter was great. I really wouldn't have expected Arthur to be the first one to get discharged! He always seemed like he was one of the sickest patients. The scene with Feliciano I thought was really well done. It showed how a visit could be extremely upsetting without anything overly dramatic happening. Lovino is a very relatable character for me because I also tend to lash out at people when I get sad. I'm also very curious to know what's going on with Elizabeta!
If it takes another year, I will wait patiently for an update. I'd just like to know how everything turns out because I have really gotten attached to your characters.

Hm... is there any chance, since it's all plotted out, that you could do a final overview chapter of how everything would have gone? What it would have turned out the diagnoses were? This fic had such a good run, but if you don't feel like you can keep writing that's of course your prerogative. But as a reader, having all these loose ends, not knowing how it would have ended... that's my biggest sadness over abandoned stories. I can't regret having read such a wonderful fic, but I do wish more authors would sum up what would have happened when they lose interest.

I have been waiting for an update about this story and I am so happy this chapter appeared today! Arthur is going to leave now? Elizaveta has a child? Gilbert will try to fix his relationship with his grandfather? I really love this story!
