Midoriya didn't break his arm the first time he used OFA - he just broke both of his legs as he rushed forward to attack the Zero Pointer. It was a couple of seconds later than he broke his arm (or did All Might and Midoriya leave that detail out when they were supposed to be telling Gran Torino everything).
