Yeah, I don't honestly think that's how Sakura would have reacted towards Naruto accidentally killing Sasuke. She's above such pettiness, and she does care about Naruto pretty greatly as a friend. So while the premise was initially interesting, that little bit about Sakura vindictively severing Naruto's hand killed my interest.

Hmm, cant really see anyway Naruto could be in physical danger at all unless it is by his own hand or unless you bring in more Otsutsuki clan characters.

Honestly, i would say the only real conflict/struggle in this will be emotional/mental. Thats not a bad thing really and will be intetesting how failure will effect Naruto, worse not only did he fail but he broke a promise.

I honestly don’t believe that’s how Sakura would’ve reacted to Sasukes death. Especially naruto being the one that killed him, I think she would’ve understood. After all in my personal opinion she didn’t even really love Sasuke... she did barely know him... But it is your story and it’s very intriguing. Look forward to next chap...
