There is absolutely no fucking way that a hive city could be taken by a bunch of barbarians even temporarily. The local PDF would massacre such a force with an ease that would make true guardsmen laugh. Hive cities are built like fortresses and are nigh impossible to take and hold, this is why the war of Kreig lasted five hundred years even though the Imperial side had only one city to its name. If the massive armies of 40k factions can fight for decades over a single city, even one occupied almost entirely by "civilians", than a bunch of backwater idiots would be massacred in seconds no matter the circumstances. Every single room, hallway, and sector of a Hive city is a killing field waiting to happen, murderholes and entrenchment as far as the eye can see; the sheer attrition of trying to take a huge city would be utterly unmanageable by these savages, even if you ignore their sheer inability to withstand the weaponry of even the most backwards hive PDF. While the arrival of asartes or cogboys or whatever would undoubtedly shatter them quicker, it wouldn't even be necessary. A single Asartes could conquer this entire world, a chapter would sweep the place in hours; the primitive weapons of a medieval realm could never so much as scratch power armour and even their magic pales before that of a true psyker's wrath. Even with only the sledge hammer of the Imperial Guard, they would sweep the world like an unstoppable tide of inglorious death, utterly unhindered by the mightiest warriors of this nothing of a world until all heretical life has been wiped clean and the world has been subjugated thoroughly.

One thing to note about fantasy is that when people and creatures are cleaved in two, it is oftentimes far more common than say assault rifles and cannons. That doesn't mean that they are desensitized to the act itself, but at the same time it's the brutality of it all. Take the Flesh Tearers for example, these space Marines are known for just that, carrying big two handed blades of every sort of on both hands when power claws are involved, the shred both flesh and armor at the same time. With chain weapons you are cutting things even though you were already cutting things, but when you are some average joe, standing there as these people turn your friend into 8 kind of chunky salsa, you tend to not want to right. On another note, I don't know if you agree or not, I personally personally think that Powerwolf works well in this chapter, I'm not going to say which song but I will say that it mirror some feeling I have for our brave guardsmen and women serving on this feudal world.
