I said this was going to be the next story to be reviewed.
Okay, now you also confirmed to me that your medical skills are real - in truth it was absolutely obvious, really, but a reader always asks.
Maybe it's because we have a fairly delicate history of cancer in the family, or because consequently my mother has a real terror about it, but this story struck me in a particular way.
It's in my top three of your fanfictions.
You are so, so good at representing the absolute, perhaps even excessive, made by an abnormal childhood, bond between Sam and Dean.
Kripke would be proud of you.

So, a very "heavy" story.
The first doubt was heavy; the anguish of the tests was heavy, and the knowing/not knowing that the diagnosis would have been almost certainly that.
Heavy was also the absence of John. Paradoxically something serious like him that in a fit of drunken anger strikes Sam, in the general context ends up in the background, while it is serious in itself. For Dean it must have been horrible. He loves and idolizes his father, and seeing him hit Sam - so being almost forced not to idolize him anymore, because nobody can hurt Sam - has led to further loss in his life.
Even if in the end, between a thousand seas and mountains, at the end of the day what really matters for Dean is that there is Sam. And of course it's the same for Sam.
Cancer was diagnosed early, and that's important... but of course, we still remain with the agonizing doubt that Dean could still die.
An ending that can be an ending, of course, but it could also continue out of pity towards the poor readers

I'm still kinda mad that it ends here.

Not everybody dies of cancer. My friend's grandmommy has cancer. She's being treated for 23 years now. And she's still alive.
