This here is a great way to start with the suspence. Now is a great time to begin the mystery if what happens is the same as the game or something different. Great to see more from your story. :)

I love how you make the story feel like Josh has an option. Even more amazing, Josh is supposed to be the less romantic character by what the wiki says. Instead, your doing a great job developing Josh and his potential for romance. I'm pretty excited to see what's next.

I wanted to say that I like your backstory, especially the part where the Psycho tries to dominate Josh's thoughts and how his illness ropes the Twins into this mess.
And the relationships of the friends are a nice way of adding light into Josh's world after the tragedy. Of course the whole Jam part is the best (they are my OTP for this fandom, hands down) *0*
I really hope that Josh will get his happy end after all, since we know what will happen next *shudders at the thought*
I'm looking forward to reading more of this fanfic :)
