Female!Tsuna fics can be interesting for more than just a change in body parts, the way society (especially Mafia in this case) sees them and what changes there are in upbringing, relationships, etc... I have a headcanon that the Ottavo Vongola, being the only female Don in the entire history of the Vongola Famiglia until Fem!Tsuna, ruled with an iron fist. Before Daniela died, she met her only adopted grandson (who is also her favorite grandchild) and made such an impression on Child!Xanxus that he decided that she's his role model, hence why he has such a ruthless personality (it wasn't just his background in the slums that influenced him).

From the looks of this fic, I imagine that if and when anyone in the Mafia World meets Yanki!Tsunade, they'll be like "Oh Heaven have mercy on our souls, it's the second coming of the Ottavo! The Mafia World is doomed!"

(And Xanxus, after the Ring Battles, is in the background with the rest of the Varia cheering her on.)
