It is not tradition for the student to kill the master in Sith culture. For example, on the Dark Council during the Great Galactic War there was a master and his former apprentice simultaneously. The killing the master thing came as a result of Revan's Sith holocron (possibly one of many) which convinced Darth Bane to trick the other Sith Lords at Ruusan to use the Thought Bomb without telling them it would kill them, too. So, at the end, Bane was the last Sith and he instituted Revan's suggested Rule of Two.

Considering both that the Rule of Two gradually weakened the Sith to an extreme degree due to indirect ways of killing the master being frequently abused and the fact that even as a Sith Revan was secretly still loyal to the Republic and dedicated to destroying the Sith, it is safe to say that Revan's holocron tricked Bane. Especially since he claimed that the ancient Sith had the same practice but on a larger scale. Somehow Bane had a major brain-fart and did not realize that if the holocron spoke truly then the Sith would never have grown in the first place. Plus, we know from the rest of the Expanded Universe and the still-canon KoTOR, KoTOR II, and SWTOR games that he was lying through his teeth to Bane.

So, yeah, best trolltastic Just As Planned ever. Revan is officially and canonically now so awesome that he destroyed the Sith thousands of years after his death with what amounts to a diary entry.
